Centrelink forms online save you time
If you receive Disability Support Pension (DSP), you may need to get your doctor to complete a medical report at certain times, like when you are doing a new claim or have a review for your DSP.
Your doctor can now complete and submit medical reports for DSP electronically instead of filling in the paper form. They will need to be registered with Medicare’s Health Professional Online Services (HPOS).
The Medical Report – Disability Support Pension form can be completed and submitted online by your doctor. They can even save a report that is not finished for up to 90 days if you need to go back to see them or get other information.
When the doctor submits your medical report, it is sent through a secure channel and automatically links to your Centrelink record. Your doctor can print the completed form if you need a copy or to keep on their records for you.
Using the online form saves you time, since you don’t need to return the form to your Centrelink Service Centre. If you need a Centrelink Medical Certificate, your doctor can also do this online from December 2011.
